By completing this form, I agree to release my academic and student discipline records to the Office of Student Engagement and Leadership Development, as well as the National Headquarters staff, as requested/required. I understand that these records will be used to determine my eligibility to be a member of a fraternity/sorority chapter.
I understand that if I provide the incorrect GPA/QPA on my recruitment application, the correct one will be substituted and submitted for use.
If I affiliate with a chapter I know that I will be required to sign a separate form to release by grades to the chapter during my term of membership, to be shared with the National Headquarters and institution as a requirement of membership and continued membership.

Hazing is NOT a requirement of ANY organization. "Pre-pledging," pledging, "lines," or any other activities that are not approved by the Office of Student Engagement and Leadership Development and/or by the National Headquarters of each organization are STRICTLY prohibited.
I understand that a payment of $25 is required to enroll in the recruitment process. This is a nonrefundable fee.
